Frequently Asked Questions
Can a rotator cuff tear heal without surgery?
Yes, many minor tears improve with rest, therapy, and medication—surgery isn’t always needed.
What are the early signs of a rotator cuff injury?
Shoulder pain, weakness, and trouble lifting your arm are early signs you may have a rotator cuff injury.
Who is most at risk for rotator cuff tears?
People over 40 doing repetitive overhead work or sports are most at risk for rotator cuff tears.
Rotator Cuff Treatment

Rotator cuff injuries are not just seen in athletes like pitchers or tennis players. They are actually more common among people over 40 who do jobs that involve heavy lifting or frequent overhead arm movements. This includes construction workers, painters, and carpenters who place constant strain on their shoulders over time.
These injuries occur when the tendons in the shoulder tear—either partially or completely. Tears can result from a sudden accident or from wear and tear over the years. As we age, the tendons naturally weaken, making them more prone to damage from everyday activities.
Although many people assume that rotator cuff tears always require surgery, that’s not always the case. Treatment depends on the cause of the injury, your level of daily activity, and what you hope to achieve in your recovery. A personalized approach can help you manage pain and regain function with or without surgery.
Causes
Rotator cuff tears usually happen due to either trauma or overuse. A traumatic tear may occur from a sudden injury like falling on an outstretched arm or lifting something too heavy. Overuse-related tears develop over time from repetitive shoulder motions, such as painting or throwing. As we age, our joints receive less blood flow, making it harder for small injuries to heal, which can lead to bigger problems
Symptoms
The main symptoms include:
- Weakness in the shoulder while lifting or rotating the arm
- Pain at rest or when lying on the shoulder
- Discomfort when lifting or lowering the arm
- Crackling or popping sounds during movement (crepitus)
- Pain may be mild or sharp, and tasks like combing hair may become difficult
- A snapping sensation may be felt at the time of the tear
Most mild or moderate rotator cuff tears can be treated without surgery. The treatment focuses on relieving pain, reducing inflammation, and restoring function. For more severe cases or complete tears, surgery might be required. The treatment plan usually includes:
- Rest and use of arm slings
- Pain-relief medications
- Steroid injections to reduce inflammation
- Physical therapy to regain strength and mobility
- Arthroscopic or open surgery for full tears or unresolved cases
